A broad scope of medical, dental and behavioral health services are offered to people of all ages, infants to seniors, individuals to families and island residents to visiting guests. Medical Services offered • Preventative medical care and health-screening services • Care and management of chronic conditions • Pediatric and geriatric specialties • Immunizations, TB testing, STD testing • Employment and sports physicals • Behavioral health assessment, treatment and/or referrals • Women’s health and family planning • Smoking cessation program Dental Services offered • Full comprehensive and restorative dental services provided to children • Emergency dental services provided to adults Additionally, WHCHC offers translation and culturally sensitive services, health education programs, and assistance with health insurance cover, either through our eligibility assistance program or through the federal Affordable Care Act program. A variety of educational topics are featured with a few being chronic disease self-management, family planning, and the care of infants and young children. WHCHC also offers a progressive approach to being available to patients needing extra help with managing their health outcomes, by participating in a patient-centered medical home program. It is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into “what patients want it to be.” Medical homes typically lead to higher quality care at a lower cost, and can improve patients’ and providers’ experience of ca re. West Hawaii Community Health Center also believes in integrating the various disciplines of health care, and systematically assists primary providers with patients who could benefi t from behavioral health services, as well. The program has received national acknowledgment for its success and has achieved NCQA Level III recognition.
