OSHA says violations by companies, Navy led to worker deaths PEARL HARBOR — A federal agency says workplace safety violations led to the deaths of two workers at Pearl Harbor last December. Hawaii News Now reported that the Occupational Safety and Health Administration has proposed fines for two companies for the deaths of two men when a 7-ton buoy fell toward them. The agency also accuses the U.S. Navy, which owns the ships, of three serious safety violations. It will not fine the Navy because federal agencies don’t fine each other. OSHA regional director Jeff Romeo said the workers were directly underneath a heavy load, which violates federal regulations and industry standards. Museum finds photo of Winchester native, submariner WINCHESTER, Ky. — Researchers at a Hawaii submarine museum have located a photograph they were searching for of a Winchester man who died when his submarine sank during World War II. Jim Converse is a volunteer researcher with the USS Bowfin Submarine Museum in Honolulu and the website OnEternalPatrol.com. He told The Winchester Sun last week that Oliver Prewitt Tipton was the only member of the USS Dorado’s 77-man crew for whom a photograph had not been found. After calling local historians, librarians and officials, Converse located a copy of Tipton’s death notice in the Lexington Leader. Converse said the notice appeared in Lexington because Tipton’s wife was living there with her father while Tipton was serving. Tipton was a Winchester native who enlisted in the Navy in 1942 and was an electrician’s mate third class when the Dorado went down. He died Oct. 12, 1943, when the USS Dorado went down south of Haiti while on transit to the Pacific. All 77 men on board were killed, and military officials never determined why the submarine sank. The photo that accompanied Tipton’s death notice has been added to a memorial page on the museum’s website. It pays tribute to the more than 3,600 men who died in submarines in World War II. The museum is trying to gather photos and biographical information on all of the men, and the website says the job is about 80 percent complete. THE ASSOCIATED PRESS OSHA cited subcontractor Healy Tibbitts Builders for five serious OSHA violations and proposed fines of $30,600. The agency also proposed fines of $15,400 for three violations by Marylandbased Truston Technologies, the main contractor. Man charged with fondling boys gets 10 years in prison HONOLULU — A former elementary school supervisor has been sentenced to 10 years in prison on sexual assault charges involving young boys. The Honolulu Star- Advertiser reported that 45-year-old Cary Cantere was sentenced Wednesday after pleading no contest to fondling two boys in Walmart and attempting to lure a third boy into a bathroom stall at Costco. Prosecutors said the Walmart incident occurred just 18 days after Cantere finished probation for the 2007 sexual assault of a student at the Hawaii Island elementary school where he worked. They said Cantere fondled the 5- and 12-year-old boys as they played with toys in the Mililani store. In court, Cantere stood with his head down, apologized to his victims and said he didn’t understand what he called his “sickness.” Coast Guard assists in rescuing divers Two divers are safe ashore after strong currents pulled them eight miles east of Leleiwi Point on Saturday. Watchstanders at Coast Guard Sector Honolulu Command Center received notification at 10:22 a.m. of two divers missing from 19-foot recreational boat Tigger Too, according to the Coast Guard. An HC-130 Hercules airplane crew diverted from a scheduled mission at 10:35 a.m. to begin a search. The Hercules crew located the missing divers four miles from their original location and dropped two smoke flares. Their location was then relayed to the boat’s captain who was able to rescue them. Both men are reported to be in good condition.
